Its quite hard to find the balance between fast paced gameplay, correctly placed monsters and an interesting map architecture.

You need a lot of experience and you have to know how to make certain enemies more dangerous (e.g. by playing Arachnotrons far away from the player or a Baron of Hell / Mancubi in a small room with a few obstacles).
Mappers should take a look at the Ultimate Doom maps, they are really great.

You dont know how many wads I dropped because of crap monster placement and retarded amounts of monster hordes, containing all monsters that will eventually cause an infight and just make the player waste time waiting for the infight to stop just because the player wants to save ammo.

Another problem is the weapon placement. It depends of the player usually pistol starts each map (like I do) or if he keeps the weapons from the previous map. But its too difficult (if not impossible) to solve this problem.
