Really surprised no one has mentioned Final Fantasy 1 NES. That port is god awful. Broken spells, weird damage algorithms, and even more... I mean look at the list of things broken in this game:

Critical Hit bug
Equipment bugs
Intelligence bug
Invisible woman of Cornelia
Magic bugs
Peninsula of Power
Running glitch
