There's definitely a difference between OPL2 and OPL3, since 3 has newer features and such. Not so sure if you'd hear a difference on older stuff that was originally composed for OPL2 devices though.
Wave Blasters and Wave Blaster compatibles are MIDI daughterboards, even Roland and Yamaha made some that are comparable to their MIDI modules. It's just a simpler and easier way to get quality General MIDI for your DOS games. Most Sound Blaster 16s have a buggy Wave Blaster interface, however, so I wouldn't recommend those. Typically I go with an ESS AudioDrive.

Mhm, Commander Keen does kind of hurt the otherwise great Tseng cards. You could always go with an S3 Virge, just make sure you get one made by Diamond, since they always have clear picture quality. The Virge may suck for 3D acceleration, but it's excellent for 2D! If you combine it with a Voodoo 1 or 2, you'll have a sweet little system.
Keeping the stock heatsink should be fine for the Voodoo 3, but you should add a fan to it. That usually helps out quite a bit. A little heatsink stuck on to the opposite side of the core could help as well.

Never tried it myself, but it looks like it could be helpful if you want a strictly DOS machine.
The processor you're going with there all depends on what you want out of this machine. If you want to only play somewhat later DOS games that aren't as strictly speed sensitive, go with the MMX266 (interesting, wasn't that actually meant to be a notebook CPU?) but if you'd like to be able to play some older stuff with a little bit of tweaking, the MMX166 may be the way to go (though may end up being a little too slow on some titles if you turn off all of the caches, an MMX200 is about the sweet spot for that, I'd say)
