You can play the game entirely with the mouse if you like. The unusual controls are part of the experience, you just woke up out of a six-month coma with a new cybernetic implant installed and nobody alive to help you figure it out. Medical and Research aren't terribly stressful levels, you have plenty of time to get the hang of things before the game starts playing rough.

Use the keyboard, for your own good. You've got SZXC for movement, AD for turning, QWE for leaning, RFV for looking up and down, and TGB for crouching. It doesn't take too long to get the hang of it, and as you collect more upgrades and weapons your power scales up dramatically to the point where you're skating around slicing everything apart in a single hit from the laser rapier before they can even shoot back.

doom only surpasses it due to mods making it more like an entire platform than one game

System Shock 1. I only played it for the first time maybe two years ago and it instantly rose to my second-favorite game of all time, only losing to Doom on the basis of mod support.
