Both NBlood and GDX are based on EDuke32.
BloodGDX was the first actual sourceport for Blood. It was made with Java and used jfDuke3D's version of Polymost renderer initially - both of those facts caused quite some drama among EDuke32 devs. Now it is developed into BuildGDX and aims to be a universal source port for Build games. Initially it was based on leaked alpha code - i don't know about current state of affairs but IIRC author mentioned the plans to remove the traces of it and build its own renderer. Initially was closed source too, didn't follow on that either.
NBlood uses the EDuke32 code. It is native, as opposed to BloodGDX's usage of Java environment, and has its own map editor in development. The guy behind it also made ports for Redneck Rampage, Duke 64 and PC Exhumed (RR and Exhumed are also supported in BuildGDX). NBlood is open source and is included in Raze which is a project to make a GZDoom of Build ports (using the GZDoom code itself too).
Fresh Supply is based on Kaiser's KEX engine and is the only port so far not developed by russians. Closed source and so far not multiplatform. KEX engine itself is derived from the one Kaiser used in Doom 64 EX, it is also used in the remasters of Turok, Forsaken, System Shock, Powerslave EX (PS1 version port canned by the devs to make a commercial one) and Strife.
