They don't hurt on Pink on the Inside or Lightly Broiled, the starting difficulties everyone should be looking at when playing the first time through. Otherwise the ninjas shooting you in Shadow Warrior are worse to deal with, and they don't "ruin" the game they're in either.

None of those 'mean placements' are shooting napalm at you. At least those cultists are consistent and gradual with their bullets, as well as being much slower and predictable.

It makes a big difference if you take in a rocket launcher to a level that doesn't spawn one. Hunted from Plutonia is the best example of this: Until you beat the level, your only serviceable weapon in that archvile maze is the SSG. If you played continuously you'll have a weapon that deals SSG damage without the huge reload.
If you don't "get the feeling" of a strain on resource management it could mean you're either decent at the game or the level is neatly balanced around it, so that's good. This also inadvertently supports how much more imbalanced a continuous playthrough would be regarding resource management.
