Like in Burner Man stage, with the giant flotting robots dropping fire that takes up to half of the screen. It's almost impossible not to get hit during that part no matter how good your reflexes are, at least until you learn every single enemy placement by heart and start shooting in prediction of that.
For instance half way through that part, the layout splits and you can take a upper route for a very short moment. If you take the upper route one of those giant thingies comes hitting you super fast and the only way to avoid it is to slide backwards a split second before the thing even spawns.

There there are the centipedes in ground man stage, not the mini boss one, the smaller ones that keep coming out of holes. Most of them are super hard not to get hit against unless you know precisely how to kill them and where, so you have just enough time to make it before a 2nd one spawns.

Then there are level design when you have to go down, but can't see what's down there, only to fall into spikes or mines, and that was playing the original version, I don't even want to think of the GBA port.

I loved MM&B, but there is a bullshit like this in almost every single level. Thankfully I'm patient and there isn't enough of it to make me dislike the game, but it still hinders gameplay and prevents the game from being truly great.
It's only when my friend played the game after me that I realized the amount of BS moments.
