I think unnecessary stealth ought to be avoided in a Doom game. The action should be intact, but still have loads of atmosphere with a few quiet moments of exploration. As to the setting, it'd be cool if it was indeterminate. Like Kingpin, for example. On the surface it looks like 1930's, but it's full of weird anachronisms.
