Doom has just had ridiculous mileage for me.
Solid base game that's still fun to play, and decades of maps and mods which will probably keep going for many decades more.
Quake is one of the many close runner ups.

Romero has personality flaws, obviously, but he also has talent, he made very real contributions to Keen, Wolfenstein, Doom and Quake which made them what they are.
Carmack has an amazing brain for the technical and scientific, but he really has no talent for the artistic side of making a game.

That's fair, because there's parts of him which I don't like.
Mind that him gloating about Quake is at least partially hollow, because the entire game was a massive compromise completely contrary to his initial grand vision, he may admit that Quake was ultimately a good game in spite of the hardships of its developments, but it's not at all the game which he had really wanted to do.

I never liked the idea of him being off the hook crazy. Maybe a little bit disturbed, he's seen and done things that most others have not, but he'd still be coherent and stable.

As flawed as the old novels were, I kinda liked his personality there.
