Here's one that seems very silly, but hear me out. When I was a kid we had our NES hooked up to an old TV that my parents got in the 80s from a hotel that was throwing out their sets to replace them with newer ones. It was very fuzzy and low-quality. Because of how fuzzy it was, the FF1 Black Belt's fist looked like one solid flesh color that matched his face color and the blue line between his face and fist was not visible. On top of that, when he punched the fist in front of his face didn't move (or not that I remember), so it looked like a hand was coming out from the other side to punch.

tl;dr - I thought the FF1 Black Belt's fist was a big round clown nose because of our awful old TV.

It seems really silly now, but as a kid the NES was hooked up via RF to a small, shitty TV that had been thrown out by a hotel in the 80s. The picture was very fuzzy, and so the fist in front of the Black Belt's hand looked to me like some kind of bulbous cartoony nose, like a flesh-colored clown nose. It didn't help that when he attacked while unarmed, a fist came out in front of the "nose," which just further cemented that idea in my mind. Even now I still sometimes see the clown nose when I look at this sprite in a low resolution.

On our shitty TV I couldn't see the bit of blue between the Black Belt's fist and face, so I always saw his fist as a big round clown nose, and I saw the black line denoting his jaw line as a wide goofy smile. So for a while I thought he was a clown without face paint on.

It didn't help that his punch animation didn't move the fist out from in front of his face, so it looked like his right arm was punching out from the other side of his big bulbous nose.
