The cover for Powerslave (also called Exhumed in some markets). Some versions of this art has him firing a revolver with a big muzzleflash, others have him brandishing the machete as seen here, worth noticing is that the shotgun he carries on his back isn't in the game.

It's difficult to find a really good high resolution scan of this art, which is a shame, because it's a great cover, on par with Doom's; featuring the game's hero surrounded by the monsters of the game, brandishing his weapon. I love the lighting as well.
