I think Blood manages to stick to a fine line between graphic and silly, like other Build Engine games. Other games are far more detailed in their gore, Blood is a lot tamer. Aside from maybe the melting spritework.

The new cultists did help. I wish there was some mod that changed enemy behavior. Personally this is how I'd handle it.
>Zombies - canon fodder even in large groups
Give them an explosive variant like the coolies in Shadow Warrior
>Fishmen - if more than one dangerous in water, on land they're worse zombies
I guess they're fine as long as they stay in the water
>Rats - fucking rats
They're fine for being a little wildcard to keep you on your toes
>Spiders - basically the same as rats but they might inflict wobbly screen effect
Also fine.
>Bloaters - beyond pointless
Give the puke ball some homing effect so you can't just crouch in front of them, also make the puke into an AoE that turns the ground toxic for a while. Extra points if you can burn the puddle, turning it into your own trap
>Gargoyles - just wait for them to actually come down and shoot them in the face
Make them swoop in for an attack and sometimes stand still in the air to throw a fast moving bone at the player, just for long enough that shooting them in the air isn't a spray and pray situation
>Phantasm - AAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAHHHHHHHHHHHHHHHHHHHH
Not sure about this one. As they are now you just have to wait for them to turn physical which is boring.

I'd give them a fireball attack when they're running up to the player, similar to Cerberus's attack, keep the fire breath when up-close
>Cheog
Make the common version of Cheog a less bullet sponge enemy, similar attack pattern to the gargoyles above but with plasma attacks
>Cultists - perfectly balanced
Despite being hitscanners I think the game does a great job balancing it out so they're not annoying like the chaingunners but can still make you sweat bullets.
>Beast
Just make it so you can avoid his stomp by jumping instead of being a gigantic invisible ball of damage all around him.
And I think that's it. My dream mod for Blood.
