It's only because he was shown for a few seconds in the first episode of the series for Takashi's Challenge. After that, it was Toujima, and he was treated as the original AD. Yamada's exist was completely ignored to the point that fans just refer to him as the phantom AD. And the only comment we have on him was Abe calling him a 'waste of human skin' when he was asked who his least favorite AD was.

Only other real drama was Inoue allegedly getting wasted in passing out in the parking lot of the station the day before they were to record a studio (This was when he was the director.) And Zari, the head of translators for SAGCCX allegedly sleeping with someone on staff. Of course we don't talk about that one here because people lose their shit.
