Hardest i've beaten are 1943 and Adventure Island, but I could never beat Micro Machines so I would say that's probably harder.
I haven't beaten Holy Diver, i had a couple of stages left, but it didn't seem that hard

I've played 1 through 4 on Famicom, plus the first one on SNES and on PC-E and 3 is my favourite

1 gets brutally end towards the end, especially 8-2 and 8-3. If you can save your weapons (which is what the TAS "longplays" do) it's not so bad but of course you lose them when you die and then you're screwed. I've beaten about 320 NES/Famicom games and AI1 is genuinely in the top 3 hardest I've beaten

Now THIS was actually hard

Here is the start of the stage. The death at the end for some reason the game trolled with a ring instead of the Milk bottle that refills the food meter, it's the only time it happened, and since I had stepped on the bad egg which is almost impossible to dodge (unless you carried a weapon from a previous stage), I was fucked.
