Koizumi didn't "sneak" it in. He worked on it
by himself and presented it to Miyamoto. In doing so he made it clear how much it meant to him.
If you want something to actually blame Miyamoto for in regards to the storybook, blame him for shooting down the possibility of a followup to it by meeting with Koizumi ahead of time for Galaxy 2 and talking with him about it.

No, people are mixing two different aspects of that story.
Koizumi snuck story elements into various Mario and Zelda games for most of his career at Nintendo.
