Ninja Gaiden is far from unfair, even for the standards of the NES. Ninja Gaiden has very tight and responsive controls, and a wide toolset of abilities for the player to use. Truth be told, it's stacked in the player's favor. An unfair game would have obtuse or strange controls, or bad hit detection, or unclear gameplay mechanics. And nevermind the 6-1 throwback, it was common for games just 2 to 3 years before it to give you 3 lives and throw you back to the beginning of the game for failure. You could argue that the enemy placement is cruel, and the respawning is a pain in the ass, but I still wouldn't consider that unfair. Try Dragon's Lair on the NES for a lesson in unfair.
