It takes place a long-ass time after Dragon Quest 2. Lorasia (Middenhall) has fallen into ruin, Moonbrooke never recovered, and most everyone has forgotten about Loto. There are even less mentions of the heroes from 2.

Alefgard has mysteriously vanished from the world, as well. I imagine I'll find out more about that as I play.
