Actually that picture is an early concept of Half-Life's snark. So yeah, it's a Kafkaesque grenade that you squeeze and release, then runs around looking for an enemy to latch on and explode. If it doesn't find any in time, it'll attack the player instead.

Since it's Doom, I'm thinking it could be a tiny demon, like a captured Lost Soul, a Pain Elemental's tadpole spawn, an Evil Eye-like cursed sigil, or something else entirely.
