"Arrange" and "original" are swapped in Japan/US

Japan= Original version is easier, Arrange corresponds to the harder Western version
Rest of the world=Original is harder, Arrange correponds to the easier Jap version

which is why I don't like to use those terms and I described my playthrough as "US Hard" which is the hardest possible skill

As for the differences: in the Jap version you take around 20% less dmg from attacks. Some supplies placement are different too: there is also pretty much NO hidden ammo (ammo that is not seen on screen and that you can only find by spamming the USE button everywhere, there is nothing like that) and instead everything is in plain sight.

There is however slightly more supplies in the western version, but since they're hidden, it's very unlikely that most players will know them all. It took me more than a decade to find some of those hidden spots, like the one behind the statue

That's something I usually achieve in normal, but on hard I don't risk dodging nearly as much; one bite = 1/4 of your HP; so I'd rather shoot considering I need as much herbs as possible for the end boss of A scenario. I guess I should try to properly learn the fucker.
