Giant Cheep Cheep scared me when I was a kid. Just the fact he ate Mario/Luigi whole.

Also not an enemy, but the room before Bowser in SMW that is dark except for that disco-ball light. That room freaked me out because it's the only place (I'm pretty sure?) it is present in the game.

The Spider boss "Dancing Insector" from Star Fox was scary and intense when it would extend it's "legs" and start spinning real close to you trying to hit the Arwing. It just felt so close like it would come through the screen or something.

A lot of the shit I was afraid of wasn't really supposed to be scary so much as they surprised me.
