from the XT up until somewhere around pentium 3, cases had support rails built in that full-length ISA cards would slot right into. often integral to the front fan mount.
PCI cards were "on the wrong side of the bracket" relative to ISA and needed an offset plastic spoiler-looking thingy to engage with the ISA card supports, but few PCI cards actually shipped with one attached, the hole pattern to fit to the card wasn't standardised, and you couldn't buy them anywhere, so we just gave up and let shit droop.
