I think the first quake takes inspiration more from the tabletop game than the original stories, while also prioritizing gameplay over story akin to doom. The game may not remain completely truthful its source material but the influence is clear if you know that game. I don't think it attempts cosmic horror, just normal horror. It gets called "lovecraftian" because at least 3 enemies and a few level names are direct references to Lovecraft stories which makes enough sense to me.

In the original story, the reason the dimensional shambler is described as wrinkly is because it's actually a man wearing the skin of one. I think the tabletop game misinterpreted this then quake took even more liberties with it. It may be inaccurate but it's still unambiguous. Here's the influence for the spawn enemy from the same book, 1988.
