It would have been a huge controversy, for sure. Soccer moms were pretty resolute back then, and videogames were still subject to that whole moral degeneration stigma. Plus it's a kid, which is a HUGE hot button issue.
Standards were way more conservative, and the west wasn't real big on all these quirky Japanese customs like we are today.

And that was present even in the companies. There weren't people over at Nintendo going "Oh man, if only we could shove some kid ass in there, that'd really convey the Japanese idea of purity!"
No, that's not a thing anyone was saying.
Even Sega, who was seen as the cool edgier version of Nintendo at the time, tended more towards X-Treme radicool type shit.

Do you remember the whole Hot Coffee Mod controversy, with Grand Theft Auto? And how, for a moment in time, that was the biggest fucking deal, the only thing people could talk about because someone modded a game to find triangle boobs that had been locked off of normal play. That happened in the early 2000s.

If you want to split hairs, no, showing Ness's butt probably wouldn't have been the end of videogames or toppled Nintendo's corporate empire. They could have "gotten away with it", in the same way Rockstar Games "got away with it", although Earthbound might have become even harder to find than it is today.
But it'd be yet more controversy to a medium that already wasn't high budget, and nobody wanted that shit. Not Nintendo, not anybody.
