It's just the standard 'Very Fatal Error' box you get in ZDoom. With the message "Tried to read memory (something)", which gives me no useful information.

I'm pretty sure it's a monster, but... I've added a ton of monsters. I set things up so that you can independently control what monsters spawn and how likely they are to spawn (see pic), but I can't reliably reproduce it.

I might. I want to avoid monsters that are too Doom-y, because it is a Heretic monster pack after all. I already use a few from Demon Eclipse that are, at least IMO a bit more ambiguous. Monsters like the Hellion, Hierophant, and the Guardian. You're definitely not going to see any Arachnotrons, for example. Not even Eriance's really cool DE redesigns that showed up in Ancient Aliens.
