Genpei Toumaden is a lot of fun if you don't mind your games being a little on the sloppy side. I'm getting consistently close to beating it, so the unavoidable damage really isn't much an issue.

Give Genpei Toumaden a try. I only started playing it fairly recently myself but I'd say it's pretty hard. You have a life bar, but with only one life, and after the first stage branch it becomes a total clusterfuck of shit trying to kill you.
