Saturn had a multifactorial death in the West.
You have to consider that Sega's credibility was already damaged due to how they managed the 32X and to a lesser extent the Mega CD. If someone bought a 32X, you can be sure that that person would most probably not buy a Saturn.
Kalinske did a surprise Saturn launch (forced by SoJ, he claimed recently) which angered a lot of the retailers that weren't warned about it. These retailers would reject to carry the Saturn for the entire generation.
Saturn launch titles could be counted with one hand, while PSX had many quality titles.
Saturn's lack of launch titles wasn't a problem in Japan, since Virtua Fighter alone was a system seller, but it wasn't so in the West.
Sony was a giant dedicated to a lot of stuff in electronics, with tons of marketing money, while Sega was only a video game company.
To top it off, the PSX was way cheaper than the Saturn. Sega couldn't sell the Saturn too cheap either since manufacturing it was expensive.

JP Saturn was outselling JP PSX until the release of Final Fantasy 7. That's an "accomplishment" if you take into account that in the West, the Saturn was already losing big to the PSX the very year of its release.

Sega did a lot of dumb shit between 1994 and 1995, They should have decided to either go serious with the 32X, or cancel it and go serious with the Saturn (of course, without all the mistakes they did with it). Not divide company's efforts between both. Also, don't kill the Genesis by 1995.

Stop wasting money on peripherals nobody wanted (the Sega Activator, the cancelled Sega VR, etc), cancel the 32X if you were planning to release the Saturn a few months later, don't fuckup the release of the Saturn in the West... Of course, it's impossible to calculate the exact impact each fuckup had in the fall of Sega. They could have done things "right" and die an even worse death too.
