I found a random one here so I took a pic. Not a complete example since this one doesn't have audio (this particular cable is used for a device that has separate audio outputs), but you can still add audio to it.

1) The cable/connector that goes to your original console. This is cut from an original/replacement cable.

2) VGA/DE-15 adapter shell, $2-4. The RGBs/ground wires (and any additional circuitry) get wired up here.

3) VGA to BNC cable you can buy straight up for $4-6. Generally you want the 4 BNC (RGBs) ones for PVMs but there are ones with 5 BNC (RGBHV). The 5 BNC will work exactly the same but 1 of the connectors won't be used when connecting to a PVM. You can get creative and use the the 5th BNC connection for something like composite video or mono audio if you want. 6 BNC connectors exist as well which are great for doing RGBs+stereo audio but they are harder to find.

0) Buy another one of 2) and recycle the cut off half of to still have the ability to use A) with original outputs like composite RCA/s-video.

If you find that you need stereo audio for a lot of consoles, you might need to make a custom with 2 dedicated BNC/RCA connectors for audio. What I did at one point was make a bunch of cables for each console which plugged into a high quality vga switcher which you can find very cheaply (pretty much beats any SCART switcher you can find these days for a fraction of the price). With a VGA switcher, you only really need 1x cable which connects to the switcher's output and can be used for both RGB and stereo audio.

Depending on the console/cable sometimes you will have to crack open the dongle to get access to the right pins if there's no dedicated cables for it by default. Sometimes you can buy the correct connector port/dongle things off ebay or other VG part supply stores which makes things less annoying. If you like modding consoles you can replace an output port with something like a mini-din.
