My guess?

Some exec either looked at the box art and went "What is this baby stuff? Boys won't be interested in this!" and had an artist to make a new cover based on screenshots or having them play the game for about 10 minutes.... or without even looking at the cover or game an exec just had an artist just look at screenshots or play the game for about 10 minutes and make a new cover.

Back then games were seen as toys for little kids and nobody gave a shit about the original story, what is considered canon, or even the original designs. Look at the abominations in Captain N.

In some cases they would intentionally alter it to make it more "American", because they thought whatever they came up with would sell better, or because they thought the original Japanese version would not appeal to customers in the US. Again, nobody cared about the story or canon of these "little kids toys" back then so they didn't put two thoughts into completely tossing the original version aside and making up whatever they wanted in it's place.

Ignorant dumbass, he's been a child and depicted as a child in the official Japanese art for decades. X and Zero are the ONLY Mega Man MC's that look like adults.
