The painelemental can become useless if the area they're being fought allows you to easily block their view or get into melee range: They don't spawn souls if they don't have a line-of-sight to their target and they have no melee attack.
Then there's Doom 64 elementals. It's not enough to make up for the lack of the other monsters but they are still great at inducing panic. They also have my favorite design in the series because I always thought the arms made them goofy t. totally not an armless caco
