Clocktower on the SFC (never got an official release stateside, but TimeWalk makes a fan translated cart, and there's translated roms) is my favorite Survival Horror game of all time. In most other horror games you have some way to fight back, but not here. The way they make you feel so completely helpless in Clocktower is just masterful. It's just you locked in with a handful of crazies. Basically the entire game is exploration and puzzle solving, and you will die, repeatedly. It also has, IIRC 7 different endings that are marked off in the menu as you get them, which is a nice touch for such an old game. On top of this the sprite work is very pretty.
