There exist three and four barreled break-action shotguns. But I suppose you could attach four pump-guns like that.
It would be unwieldy like all hell, and something like an USAS-12, Vepr-12 or Saiga-12 would be way better in all general regards, but yeah, you can do it.

That wasn't a factory four-barrel though, that was a pair of double-barrel shotguns attached to each other (and then sawed off in a really weird manner).
