It's more difficult, even japs complained about it.
Enemies and Bosses have ~25% more HP and do ~25% more physical damage. Magic is nerfed.
Enemies and Special enemies drop 3x less gold.
If you know where to go and what choices to make, the moon runes won't be an issue.

Another reason why Lenus was a difficult boss was because of the savepoint.

Once you were in the castle, you were trapped until you beat her and many first time players only used 1 save slot.

I know I had to restart from the beginning because of this.
