Deathlord. This hardcore RPG was only on the Apple II and C64. Day/night cycle, seven huge continents, and like the OP game, permadeath that gets written to your save file. Considering the size of the game world, the whole thing also fits on one double sided floppy.

Deathlord aka the meanest RPG ever made. No console RPG is this hardcore.

Deathlord. This game is Wizardry IV levels of make you want to cry. Especially because when you die, it automatically records it to your save game, so...game over. You have to restart from scratch. Keeping about 50 backup save copies is necessary to survive.
