I had one for the PC back in the day, it was made by Interact, and looked exactly like image related. came in the same box too. There is also another PC variant called the Mako PAD. I love how the analogue stick advertises "awesome for flight sims" , because not a lot of PC games actually supported analogue movement, back then, outside of maybe flight sims that supported flight yolks, and analogue controls. Never owned the N64 pad, but given that Interact made that one too, I would imagine it is mostly the same.

Interact also made a version of this game pad for the N64 as well.
