Probably nothing, at least in the US market. By the time it came out I think the Saturn had tarnished Sega's reputation a bit too much. A second analog stick might have made it a bit more appealing and allowed some additional ports that could have given it a bit longer lifespan.

My Dad was heavily involved in the tech industry in the mid-80s to the mid-2000s, and I remember when I was a kid one of his friends that was doing some side work for him was working for Sega at the time. He would be over at the house frequently and at some point asked if I was excited about the Dreamcast, to which I essentially replied "..Eh". I actually remember watching his face drop when I said that, and I still feel a little bit bad about it even all these years, because when I finally did get my hands on one I actually really enjoyed it.
