Hideous Destructor sounds like something you'd like. The chaingun sprite is scaled down, but that's the only visual change. Functionally, the weapons are much more realistic. Bullets are projectiles (fast enough that it's not really noticeable, but still), manual shotgun pumping complete with shortstroking if you suck at it, automatics have their speed massively increased, with the chaingun's default firing speed increased to 700 RPM and the option to switch to 2100 RPM. The plasma gun isn't just a boring projectile spam weapon anymore, instead it allows you to SMITE YOUR ENEMIES WITH LIGHTNING BOLTS. The shotguns have very good range, with the SSG practically serving as a sniper rifle. There's several new weapons too - a ridiculously unreliable assault rifle (which you're basically forced to use because it has the most common ammo), a battle rifle that hits harder and doesn't jam or overheat like the assault rifle but has rather uncommon ammo, and a cannon that pierces enemies and kicks so hard that you'll actually hurt yourself if you don't fire it from the correct position. The sounds are a bit of an acquired taste, they're definitely not what you'd expect in a Doom mod, but I personally find them rather satisfying. And of course there's a catch, which is that the monsters are all buffed to the point of most maps becoming unplayably hard. Still, it's a lot of fun if you're willing to re-learn everything you thought you knew about Doom.

I've beaten Doom E1-E3 and I'm up to Map26 in Doom 2 with it. It's perfectly playable and actually probably one of the best-balanced mods I've ever played, it just has a really steep learning curve on account of having to re-learn basically everything you thought you knew about Doom, even things as simple as shooting enemies or healing wounds. The enemies are among the smartest I've seen in the Doom engine, and they're all equally deadly. It's a ton of fun once you get over the learning curve, and with a buddy it's the best survival mod ever.
