It's usually good to come up with of a basic concept and a theme for a level first. You can start by sketching it out on paper but a simple mental image does fine as well, whichever suits you really.

Some people just wing it which honestly isn't that bad of an idea. I've found that concepts for different room layouts come more naturally this way. Helps a lot if you don't know where to actually start.
