Make a shortcut and add -w to the target. It really doesn't want to run in fullscreen on modern systems.

Alternatively you can grab the glide wrapper at http://www.svenswrapper.de/english/ and use that to run it in fullscreen. It also has some other goodies but some people prefer the other graphical modes over glide/3dfx since they look slightly different.
