Astronomers can approximate through a mix of methods. We can:

1) Extrapolate the spiral arm structure to the other side (doesn't provide much accuracy but it's something)

2) Observe exceptionally bright stars on the other side that penetrate through all of the interstellar dust (see http://www.scientificamerican.com/article/a-stellar-discovery-on-the-milky-way-s-far-side/))

3) Infer about what we can't see by observing the other side's gravitational interactions with what we can see.

I'm not entirely sure how well-used #3 is, so you may want to fact check that. Google around and you'll find tons of interesting articles about the galactic structure.

Pic is a view we're missing out on by not living on a planet within our satellite galaxy called the Large Magellanic Cloud (LMC). If an alien civilization exists up there, they evolved with this dominating night skies at about the brightness of Mars but with an angular size larger than 70 full moons.
