Why not take organs out of the body,clean them or if needed repair or replace them,and then put them back into the body?

If you heart is clogged, then just have doctors take it out and clean it.

if you remove the organs you reduce the chance of plaque getting into the blood stream, and potentially causing a brain damage.

you mean like atherosclerosis? we used to do that (have an operation where a surgeon would scrape the plaque out of your arteries) but it didn't change the health outcomes because the plaque grows back quickly.

This. A lot of organ diseases stem from unhealthy behavior. So surgery isn't going to help.

Also, major surgery like that is full of risks. MRSA, tissue death, heart failure (it doesn't always restart), blood loss and so on. On top of that is a tremendous recovery time and a monstrous medical bill.

If it's possible to get the same result by stuffing a pipe up through the femoral artery, that is much preferred.

That's the thing, they're still working on it. Problem with transplantations is mostly that the risks outweigh the benefits. Although some crazy advanced surgery is definitely possible, the risk of dying during a heart transplantation is still higher than the risk of dying from atherosclerosis.
