It is obvious that our brain can work with an equivalent of "pointers", well-known concepts in programming.
I can think about "cup" without thinking about cup. There were several shit-threads about this recently.
While in the hospital, it is possible that she created pointers to "this room", without actually defining "this room". Later, "this room" was filled in with the actual room. To her, it seems like the memory always contained that room. Even though it really didn't.
This happens in dreaming too, a lot.
Never had a dream where you've realised that some object or person in your dream was actually a particular object or person all along. I can imagine that a near death experience is a lot more intense then dreaming, and that the usual metnal safeguards are not in place/
