Can two unique systems evolve to share the an indistinguishably similar state? i.e. Are our physical models surjective?

got your terms mixed up bro -- surjective means every element of the target space is hit by something. If your domain is "unique systems" and the codomain is "things they evolve into," two evolving into the same state doesn't violate surjectivity.

As a simple analogy I mean, can we have a physical system which can be described as 2+3 -> 5 and another which can be described as 1+4 -> 5, such that if we only knew about 5 we couldn't know if it's previous state was 2+3 or 1+4?
I may just be stupid, but this doesn't seem obvious to me.
