These would be my ideas for improving 4chan moderation:

1. Create new reporting category for NSFW images on SFW boards.
2. Create new class of subjanitors with the power to delete NSFW/illegal images and to set/unset off-topic threads to permasage. Recruit lots of them.
3. Extend the flood limitation so that after starting a thread you have to wait to start a second thread on another board; this will greatly reduce spamming.
4. Allow janitors to give out 3-day bans for IPs who make 3 or more spam threads within 30 minutes.

Permasage is a good idea, but it is currently executed wrong.
Although the mod fighted anti-bio threads once this way, I've also seen him abusing his power to autosage legit threads that he personally disliked.
A better concept could be some democratic report system. If a thread gets a certain number of reports from different IPs, it gets autosaged.

Examples of the kinds of threads you're speaking of?

I don't think any threads would ever be autosaged if you required reports from 20 different IPs. Most people don't report at all, and the captcha makes it even more not worth their trouble. On a slow board like /sci/, you might be lucky if one person reports a thread.

OTOH, perhaps more people would report posts if it did something.

That said, finding 20 proxies through which to report a post is not all that difficult.
