Up to the 19th Century it WAS fashionable to have a 'healthy' women. Then after mechanisation really set in the 20th Century saw that change.

Once people in the West effectively eliminated child mortality, danger giving birth, and famines, it became fashionable to prove that you "didn't need" the attributes that previously ensured survival.
