I may very well have failed my step 1 (bounced around from passing/fail scores during dedicated).

I need to come up with a plan in case I do. I don't want to go on full crisis mode. I wanted to be a surgeon but I know if I did fail it (it's p/f now) that I need to be realistic. Please please advise.

School isn't primarily about learning, it's primarily about proving that you're able to cram arbitrary things in your head. And in a distant second, it's about learning.

The rational move would be to put in the effort in other subjects to show them that you're capable. It also doesn't hurt to be well-rounded, the other subjects aren't irrelevant at all.

Yeah I do do that and do appreciate being well-rounded, in fact I believe that the significance of neurological factors in cognitive disorders is egregiously understated, it's just that without being in a small engaging environment forced to participate in these less-relevant classes which I had in high school, it's exceedingly hard to motivate myself to actually engage in those classes.

Just doing all that basic memorization feels useless to me because I know when I'm motivated I'll remember everything and when I'm trying to solve a research problem and learn about something I pick up on all that basic stuff naturally anyways. What I did was basically give up, because I was only innately interested in the things that I was passionate about, and I had a really hard time finding the motivation to study anything else.
Turns out, in retrospect, I've the olde ADHD and that explains a lot. (You might not have it.)

I think you should bargain with yourself and decided how much effort/pain you're willing to put into this. You don't have to go for the 'optimal minmax' if it'd make you miserable.
It's also okay to do what you're passionate about, coast on the other topics, and try to get away with it. As long as you're talented in general, you'll most likely be fine. Worked for me, I barely studied a single thing in school that I wasn't already interested in.. I just went to a meh school, got my diploma, and called myself self-taught while putting my non-classroom projects forward

The bar for ems is definitely really low, but I think there's a lot of politics regarding raising standards and knowledge. The NREMT is already super easy to pass and CE standards are really low. As long as the bar is low, scope will be gatekept by a year+ of schooling. Despite that, most of the emt firefighters I've met seemed like they could handle something in between BLS and medic.

I used to work for a private ambulance company doing mixed ift and 911, but I'm going back to school to get my RN, so I let both my state and national certs lapse. I've only worked in a major metro area, so it was pretty rare to see a fire crew without a medic. It sucks that medic school is such a big investment if you don't already want to be a firefighter or something niche like air rescue.

Diet is bad, I have corrected it a little and am cooking more.
Right now my diet has been very low fat because I've been eating chicken and vegetables because I have them available and am trying to save money and not buy a lot more groceries, so maybe the lack of fat content could be wearing me out?

I'm not sure why nobody will just test you for ADHD, but if at all possible the official formulations would be much safer and more reliable of a dose. Both due to reliability of manufacture and assurance against that huge spike. My understanding was that a major contributor to neurotoxicity was hyperthermia, so I try to combat that. Wikipedia also mumbles something about vit D maybe being neuroprotective for methamphetamine, so on the off chance I figure that shouldn't hurt.

I stayed away from any fast uptake mostly because of the ΔfFsB addiction concerns. I want it as smooth, slow, and even as practical, so that I can get stuff done when I fall behind. (If I wanted to do drugs for fun, I have much safer ones to do..)

I did get tested for ADHD, unfortunately in my country only MPH is approved. Amphetamine stimulants were my reluctant plan B in case MPH didn't work (which, sadly, it really wasn't enough to help me not fall behind by 6 month or a year on important things..)

Absolutely. For obvious reasons, taking street metamphetamine for ADHD is an absolutely alarming idea, and I do not recommend people try to do the same.
Not visible are the long hours I've spent considering various options. I normally like to be very skittish, careful, and exceedingly prudent. Obviously, this is an extremely reckless sounding idea, but I've arrived at the unfortunate conclusion that this seems to be my best option.

I'm thankful that I've never shown any addictive tendencies. I'm very introspective, so despite worrying a lot constantly, I'm also fairly confident I don't have any signs of addiction, or any desire at all to take more than my future self would approve of, looking back on my current decisions.

Depends what you took. I'm not an expert on cooking with laundry, so I'm not sure what exactly they put in those.
But for instance, you can actually drink a glass of household bleach, and because it's typically low concentration (3-6%) it doesn't really do that much damage.
This is also why suicides by household products or overdose fail so much. The stuff you have at home is generally pretty mild, unless you're really making a science out of trying to injure yourself.
