If people would have aches due to posture and muscle under development, proportional to how bad their balance is compared to a more healthy person, they would cry all the time.

If unhealthy people would feel nothing at all for carrying disease that can be passed on, they would not be weeded out by evolution. (And those creatures who lack this will die out because of their weakness.)
