There were some possible issues perceived with this, so a SDV was proposed to handle large missions that would otherwise need to be broken up across multiple launches. Given the time-frame something like this would have either been used for a prospective lunar program or SDI.

This is a late 50s or early 60s R7 variant with an ammonia-fueled nuclear core stage. It was expected to lift about 40 tons to LEO, and if they felt they needed it, they would have built it. The Soviet's wouldn't have given two shits about public opinion.

These days you'd have to look to China for something like this. Their superheavy-lift and reusability programs seem to be progressing at an acceptable pace, but if Starship gets too far ahead of everyone else there's always the chance they'd reach for something crazy to close the gap some.

They've had a good record since B5, especially considering it's only being done for cost savings. Because the Angara A5 costs three times as much as the Proton-M it's supposed to be replacing, and the Soyuz-5 could launch the Orel for a fraction of that. Roscosmos has to be excessively price conscientious right now, and it needs a something that doesn't cost over $100 million per launch whenever they want to send crew to a station. Trying to run the cosmonaut program on Angara-5/Orel would be like ditching Crew Dragon for a Delta IV Heavy launched Orion.

The bigger problem is that Soyuz-5 has no launch pad. They don't want to fly from Baikonur because renting that graveyard is costing them $115 million a year and they want that to stop. They can't launch from the Zenit pad at Plesetsk because that got converted over to launch the Angara, and they can't build a new pad at Vostochny because the trans-Siberian railroad can't handle objects with a diameter of more than 3.8m. That leaves they with renovating the old Sea Launch Odyssey platform, and that's been rusting out in Vladivostok for years.

The Angara URM-2 is only 3.6m and the fattest a Soyuz segment gets is 2.95m, so it was just easier to not make a big deal out of it when the alternative was suggesting rebuilding every bridge and rail tunnel between Moscow and Amur. Building the rockets on-site wasn't going anywhere either, since you'd need to build up a huge amount of infrastructure from nothing and then build the city for all of the factory workers to live in, also from nothing.
