In the early times earth will have limited control over distant settlements even within the solar system due to the vast distances and slow communication and transportation. Just as the Roman Empire was also limited in its time.

For awhile earth will be content to absorb profits from the companies they invested in which exploit space. It won't last forever though and eventually they will start to take more by force. Such is the nature of things.

Even with FTL I can envisage methods by which a covert culture could avoid detection. I never believe any of this shit materializes until it's already in phase 3 trials.

For example osteoporosis is a major issue for the elderly and we still can't do fuck all about it despite bone being a very responsive tissue. We can only ever either make it worse or temporarily make it better but with issues and as soon as treatment stops it gets worse again.
