Just how much can your diet impact your mood? The difference in how I feel when I fast is unbelievable, when I eat regularly I'm lethargic, unmotivated and depressed, when I fast (one meal a day), I feel great, super motivated, optimistic and just...great. I want to say it's just placebo, but the effect is absurdly strong, and it seems at least weakly correlated and consistent.

So, my question is this: is it at least theoretically possible? Are there any mechanisms related to fasting that could cause such a dramatic change in mood? It truly is night and day difference.

>peds short/long term stay

>9 year old who was involved in a ski-doo accident 8 weeks ago
>TBI, broken bones
>will need years of physio to be back at his cognitive and physical baseline
>parents see their boy with all these cognitive deficits
>decide to stop visiting him
>parents haven't seen the kid for a month now
>they ignore calls from the hospital
>child protection is involved
>child is ready to go home and do rehab but he can't leave without mom or dad
>legal issues now has the boy essentially stuck in the hospital
>parents want the hospital to take full responsibility over his rehab and care until he's "back to normal"
>meet patient
>happiest little guy you can meet, just takes him a while to get sentences out, moves more slowly
The peds unit says this occurs more often than one would like to think. They had one kid stay for 2 years until he aged out and was placed in a long term care home.

>> No.12672834

>>12672784
Yeah peds is depressing. On top of my head I remember a 14 year old guy who got some kind of malignant brain tumor. Made it through radio and chemo to end up depressed so he tried to anhero to his head with a .22 but the poor guy survived the bullet, the craneotomy and the intensive care with half a brain only for the cancer to return a year later or so.
The cases in there make you question existence itself. Luckily I spent most of my rotation in pediatric surgery and left the grunt work to the other interns.

I see the point with peds being depressing though. Its strange remembering all us excited 1st years who wanted to go into peds to get the sense of doing good, and saving kids and shit. Real reality check when you're on the unit. I mean, peds isn't all bad. It's nice to treat conditions that aren't usually caused by some type of self-neglect like in adults.

If you're extremely interested in academic research within medicine mostly then go for it.
Workload will vary depending on your project but what I've seen is they usually do 2 years of med school basic sciences alone with maybe some basic research then their PhD for 4 or so years then finish up with 2 years of clinical. So it's spread out and not super intense but still a bit more intense than either alone.

Was he abused at all?

I know a somewhat similar case but the patient was older in her 20s. She was a cerebral palsy and schizophrenic patient who came in for severe ITP. She was on carbamazepine for seizures and eventually developed neutropenia and neutropenic fever. Eventually treated and everything was fine. Her parents who were her legal guardians refused to take her back basically saying they thought she would die so they wouldn't have to take care of her anymore.
She stayed in the hospital for months after as social work tried to find a place for her. She kept getting rejected because she was aggressive around new people. I don't know whether it stemmed from abuse but when washing her one of the nurses told me and the attending she had some scars on her inner thigh and labia.
She eventually found placement and was discharged.
She liked seeing me around. Would always wave to me and walk over with her spastic diplegic scissor gait to talk to me. I felt real sorry for her but hopefully she's better off now.

We see some terrible shit don't we.

I think most of the time non-white patients are honestly better served by people from their community. There are so many underlying social cues that a particular culture, ethnicity, or community can understand and that can affect healthcare because we're dealing with dynamic human subjects who can lie or withhold information important for diagnosis and treatment.
For example, had a young black man come in, asked him about drug use. Couldn't get anything out of him. Another black resident went in, spoke to him and was able to figure out how much of each drug he used each day which helped a lot in treatment.
But I strive to treat everyone as how I or my mom would want to be treated as a patient.

But you can't really tell anything without imaging and biopsy.
I don't want to give you false hope though or put you into despair. Just wait and hope for the best. Cancer patients who do well are usually ones that are hopeful about their prognosis and have family support. Even if the prognosis is poor, use what time you have in the best way possible.

Yes and no. I work in a heavily ethnic community. There are certain stereotypes that are completely true, no matter how "woke" you are on politics. Lots of drugs, violence, theft, gangs, kids with no futures, poor education, no jobs. I actually think it's more of a gender thing than a racial one. The young males from the predominant ethnic group are almost invariably retarded assholes who I usually only see doing sketchy shit on the streets, their kid is fucked up, or they got fucked up. It's fucking retarded because when I ask them questions like "what did the fluid look like"?, they puff up their chest, scowl, and give me a dirty "yea". Basically, no manners. The young females are usually only shit if they're hooked up with a troublesome male (often). One of our receptionists is taking a year off because of domestic problems (i.e. abuse; she came to work with poorly concealed black eye last summer). The middle age and elderly ethnic females are very much your typical motherly woman. Middle age males are generally okay, older males are almost always except for being stubborn. So it's hard not to look at the changing gang tags on the side of the clinic sometimes and not think "you're retards". I've been thinking of how to help the community, even going as far as considering offering tutoring, but I'm just not sure if a white guy doing that would be well received.

Does it affect my care? Maybe, but I try not to. I know it's bad form to, but I just see patients as a walking checklist. I do try to elicit their preferences when I can, but it's just information that goes into the checklist. So whether you're nigger, wop, kike, chink, honky, or poo, it doesn't matter to me unless it relates to specific ethnic/genetic things on the checklist. No, I don't believe in microaggressions.

Honestly not sure DO would take you. Your GPA is ass and the MCAT is not good enough to compensate.
Apply widely.

I'm around that political spectrum and that's still a worry for me. I unironically want to know more about a fascist medical system, or even the nationalist germant med system.
