Hello /med/. I am a type 1 diabetic (27, male). I have read articles about CRISPR being used to cure type 1 diabetes in mice and have identified the email address of one researcher involved in the project. I am not a doctor or medical student, but I am interested in anything which might cure me of this disease so that I no longer have to take shots every time I eat or sleep.

I think I have a Periapical cyst

I had a root canal done on a front upper tooth a few days ago. There was tremendous pain from percussion testing the day of the RC; the nerve was dead and necrotic but, the dentist told me we caught it early (It was only that day that percussion was causing extreme pain). We took and looked at xrays and he said that there was an abscess at the base of the root but I was told that it had basically just started to become an abscess and there was no need for concern or anti biotics at this time. Shortly after the root canal I still had some pain from the tooth being applied pressure but my dentist told me that was basically the (nerveless) tooth pushing against the infected area which was aggravated.

The day after the root canal I noticed there was a lump above my gum line, under my nostril where the root of the tooth ends. I told this to my dentist but said to call him back if I noticed any swelling .But I don't know what that means, this is very localized swelling, does that count? I can't find any information on what kind of swelling I should expect after a root canal.

So I think I have a Periapical cyst. But is this a good thing? Does it mean my body has sequestered the infection and is absorbing it? It's been like this since the day after the RC and its been 4 days now. I don't have any pain and besides the relatively small cyst-like formation under my nostril, I dont notice any swelling. No fever etc. I will try to get in contact with my dentist as soon as I can but it would be helpful to understand if this is normal or not

I think I have a behavioral disorder called Onychotillomania. Basically I compulsively pick at my nails to the point where they bleed. I also think I have a fungal infection which is what started the picking habit, because the nails were so easy to pick at.
I have a psychiatrist, and I have a dermatologist, but I’m afraid of going to the dermatologist with something that is rooted in my brain. I just want to have normal looking hands and feet again.
Are there doctors who make it known that they are “friendly” to mental health issues? I called my dentist and he basically said they don't prescribe antibiotics unless the situation worsens. about a year ago, my girlfriend had a tonic-clonic seizure, i'd never seen one and i didn't know what to do. i put her on the ground and moved stuff away but unfortunately i kept her on her back and saliva pooled in her mouth probably. she went cyanotic and i didn't record the time when she started seizing but it felt like 4 or 5 minutes. maybe it was only like 3 minutes.
she had labored breathing and snoring type stuff afterwards, and i carried her to the car and brought her to the ER. ran tests, MRIs, and shit and she was fine but i've been feeling really guilty about it ever since.

i failed to put her in the recovery position, and she may have had brain damage from failure to clear her airway. i knew what the recovery position was, i just didn't really think about it outside of like, a blackout drunk. i also could have done mouth-to-nose resuscitation, but i didn't think to do it either.

why is it that mouth-to-nose resuscitation is not recommended in this case? to me, it seems like it wouldn't do any harm, and you could at least force fresh air into the lungs (which is being forced out by the diaphragm contractions). the heart is still pumping, after all. why is it just recommended to put them in the recovery position?

memorize the general system, so what the qualities are from medial to lateral, everything else location wise you should be able to get from their exit points. Maybe some extra stuff like the genu nervi facialis. Word from my neurology-prof is that a lot of the specific stuff isn't that important later on as it rarely comes into play with common injuries

Doubtful that this will be useful or that relevant in the exam, more which field of vision projects into which general lobe and which fibers cross over where

Can't say it with authority, just what I've gotten from my neurological lectures, but it seems later on it mostly comes down to testing reflexes and knowing general signs of lesions. You'll usually be presented with either broad damages from trauma, or specific damages from infections, tumors or strokes. You can go pretty insane with with the extensive details of neuroanatomy, having a practical understanding of it seems to be the way to go

What? Some of the best docs are DO's. I have nurse practitioners that are better than the doctors, and nursoids run the unit and are the ones responding to a brady event and respiratory arrest in seconds with a bag-mask/atropine and paging anesthesia stat while the doc shows up afterwards and says "well damn". Nursoids run the show on these ICU's, the only bad ones are the ones that don't know when to quit playing doctor and get the actual doctor.

Doctors can bitch all they want but when I have to move them to the side and fix the EVD to get an accurate ICP reading they sure are appreciative.

How bad is covid in your hospitals? DFW here and my hospital is basically one of the big three here and we're 3 full covid ICU's, two out of the three PCU's are covid (not to mention all the med-surge covid floors) while the other is PCU is ICU overflow. We have poor staffing, regular Three to ones when one of your pt's should be one to one in ICU's and now we are considering switching to two pt per one room. ED has called safe harbor twice in the past two weeks because they had 12 nurses to a 90+ bed ED on hallway protocol. We can barely keep one ICU bed open if that, I think the numbers are 40% covid+ pt's in the hospital, 60% of ICU pt's are covid and 50-60 covid vents. It's a nightmare

Had a pt present w/ progressing weakness, AMS, hemiparesis (no history of MS), FLAIR symptoms on MRI etc. I'm thinking autoimmune, so we run our CSF cultures, mayo clinic viral panels etc. In the meantime we successfully extubate and my RN came to me saying he's officially neurologically declining. So we re-intubate, get the MRI which shows significantly increased FLAIR and things are coming back negative on the CSF and I realize "holy shit the doc that first took him started him on abx and antivirals quickly, I need to run some blood panels from blood that was drawn prior to med administration (and repeat LP's etc)". So my nurse, albeit new, was luckily a very competent individual and despite lab saying bullshit about how they couldn't get us those samples he dug around and figured out that there was blood to be tested.

Family is two intelligent college aged kids who lost their mom to seizures with a lot of outside pressure as this pt is stupidly rich, so he has a fiance he dated for a year that likely was after the money per the kids, and they are being pressured by his friends for "second opinions " even though there are multiple competent docs giving second opinions every shift. Luckily he's cool, reviews the case, and just recommends I drop a quinton, consult nephro, and start PLEX treatment in case of autoimmune and he'd follow along.

So of course this blood doesn't show much of anything and finally some CSF cultures come back and lo and behold it is just herpes encephalitis so he was fucked. Was tough giving the kids the poor prognosis talk and watching them come to realization of what was about to happen. Luckily the nurse told me he had done "the ole DNR talk and please talk to the Dr. about it at rounds" so he had them prepped for me to explain the benefits of DNR based on prognosis. After that it took a few days of the poor kids having to sort out family dynamics and we withdrew.

We intubate for intractable seizures all the time and start burst supression (where we basically keep your brain activity to a minimum w/ sedation and restart your brain like a computer)

For regular seizures we can medically manage it and ER/Neuro nurses are competent enough to manage the airway. The problem is that the airway isn't being protected so it's better to just put them in the recovery position and protect the airway to prevent aspiration as ventilation always carries significant risk. Now if you're just talking mouth to mouth etc. you can bag somebody to bring their sats up and prevent hypoxia but you have to fix the primary problem which is seizures and keeping the airway clear to prevent aspiration. No point doing mouth to mouth or bagging a patient if they are just going to aspirate (which basically means sucking something into the lungs that shouldnt be there), unless they are critically low in oxygen and you're just trying to keep them from coding. You most likely would just fuck up the more important things trying to bag them.

Don't feel guilty, you're not a trained medical professional, there's nurses that don't even know to do this shit outside of neuro/ER/ICU. It's not your fault you didn't know what to do man. Next time just put them in recovery position and protect the airway. It sounds like your smart enough to do your own research so start there

I think I have a bilateral posterior dislocation of both my shoulders, that has gone misdiagnosed for over 4 years, I live in constant pain and suffering, it feels like my shoulders hinge on the wrong point of my scapula, and that I can not move my shoulder blade back and down correctly.
